2026 Fantasy Outlook
Perez was sidelined for the first couple months of the 2025 campaign while completing his rehab from Tommy John surgery, and he struggled out of the gate with a 6.19 ERA and 14:10 K:BB in his first four outings. The right-hander settled in from there with a 3.86 ERA and 0.96 WHIP in his final 16 starts, finishing the campaign with a workload of 118 innings (including his minor-league rehab starts). Perez flashed his potential as a rookie in 2023 with a 3.15 ERA, 1.13 WHIP, 28.6 percent strikeout rate and 8.3 percent walk rate, and he pitched similarly well in his first year back from surgery. There are some volume concerns given that Perez has never delivered results over a full big-league season, but he should be able to push up to around 150-160 innings in 2026 given what his workload was last year. The hard-throwing right-hander had an up-and-down year after a late start amid his recovery from 2024 Tommy John surgery, though he flashed plenty of upside for fantasy managers to take into consideration ahead of 2026. Perez will end the campaign with a 4.25 ERA across 95.1 innings, but his 1.05 WHIP, 105:32 K:BB and .195 opponent batting average are highly promising figures.

Right elbow inflammation during spring training quickly progressed to Tommy John surgery for Perez, whose 2024 season ended before it began. He'll get a late start to 2025, too, with the Marlins projecting a midseason return for the talented youngster. Perez showed fantasy ace potential at the tender age of 20 in 2023, and if there's a silver lining to him missing the first few months of the 2025 campaign, it's that the kid gloves should be off once he does return. Immediately regaining your stuff and command post-Tommy John surgery is not a slam dunk, of course, and the reality is Perez will be a better fantasy investment in 2026. Still, he could pay dividends in the second half of 2025 if you don't mind waiting it out.
